Complete the questionnaire.

Your information will be reviewed and analyzed. Some clarification may be requested.

Upon verification you will receive a certificate in English, Spanish, or Portuguese and a printout of your family name from Name Your Roots.

 

This certification is for the purpose of demonstrating one’s Sephardic ancestry and can not be considered proof of someone’s formal Jewish affiliation.
